The AHA NIH Stroke Scale, also known as the fun-o-meter, is no laughing matter, although the name does bring a smile to this nurse's face. Used to evaluate and document neurological status in acute stroke patients, it tests different abilities that could potentially be impacted by a stroke.

Facial drooping

One section tests facial movement by having the patient smile or show teeth. We wouldn't want stroke symptoms to ruin anyone's beautiful smile! Though in the moment, asking a patient to pose for funny faces seems less than sensitive. Laughter may just be the best medicine.

Arm wrestling

The scale also assesses limb strength, like asking the patient to engage in a friendly arm wrestling match. Though if their limb drifts downward, the humor may be lost. So I'll abstain from any "arm around" jokes and handle assessments with care.

Foggy speech

Slurred or foggy speech is evaluated by asking patients to repeat tongue twister phrases. While tempting to pull out "unique New York" or "red leather, yellow leather", clarity of communication is no silly game. Though a speech assessment seems like the perfect time for a one-liner!

Balance and coordination are tested by observing walking, almost like a police field sobriety test minus the humor. But while synchronized stumbling seems entertaining on TV, a stroke is no joke. So assessments will be handled with compassion, not comedy.

Final score

In the end, the fun-o-meter assigns a numerical score assessing stroke severity. But whether the tally elicits cheers or tears, we'll support each patient, celebrating progress made on the road to recovery.
